With the vast number of insurance plans, it is the patient’s responsibility to understand their insurance coverage. At your initial visit, you will be asked to complete paperwork regarding your demographic information, insurance information, and you will be asked to provide your insurance card for copying. If you do not have an insurance card available, full payment is expected at the time of service. At regular intervals, you will be asked to update this information. While we acknowledge that this may be an inconvenience, it is necessary that we maintain current information and ask you to notify us immediately anytime there is a change.
You are responsible for any deductibles, co-payments and/or any referrals that your insurance company may require. Payment is expected in full at each visit for services not covered by your insurance plan. Always call your insurance company first and verify that we are participating providers.
